I think it's a survey/a quiz.

It can't be a quiz because there are no wrong answers, so I guess it qualifies as a survey more.

1. What was the first Japanese song that you ever listened to? Daisuki Dayo- Otsuka Ai

2. Did it inspire you to like that group/singer, or did it turn you away? It inspired me to like her, and all Jpop in general 

3. What is the one song that you have been listening to for the longest time? Massara Blue Jeans/Sexy boy. I've been listening to them constantly since I got into jpop  

4. What group/singer have you liked the longest? Morning Musume/C-ute, thanks to tintin, and Otsuka Ai.  

5. What is the song with the most plays on your itunes/media player? Egao yes Nude (I've been whoring it recently. xD)

6. Is that song your favorite? Yes and no.

7. If not, what is? Cindarella\Complex and Mirai Taxi
